Quarterly reports are to be submitted on a calendar basis:
- 1st Quarter is through March 31, due April 15
- 2nd Quarter is through June 30, due July 15
- 3rd Quarter is through September 30, due October 15
- 4th Quarter is through December 31, due January 15
To complete the quarterly report process:
- Print this page to help you organize the information before you complete the online form.
- Complete the form by entering all required fields (use worksheet below), making sure all information is accurate, and submit the report.
- Fax the most recent IRB approval letter and consent form to Ms. Rhoda Marte at 301-571-0768. *NOTE: Please make sure SMRI is NOT listed as the “sponsor” of the project on any IRB document.
- If you have thoroughly read the instructions and have any questions, experience problems while completing the online form, or are unable to view the report, please contact Ms. Marte by e-mail marter@stanleyresearch.org, telephone (240) 482-3018, or fax (301) 571-0768. *NOTE: Please allow two to three months from the deadline date for a complete review of your proposal and for notification of approval or nonapproval.
Report entry instructions:
- On the “Trial Report Log On” page, enter your confidential Login and Password.
- After you have successfully logged on to your account, click on “Submit Quarterly Report."
- If you have submitted a report before and want to submit a new one based on the previous one, select "Open My Latest Report." If the system does not find your previous report, it will open a new form.
- Navigate from field to field within the application by using the Tab key, not by using your cursor or arrow keys.

When you have completed your report and you are certain the information you have provided on the form is correct, click on the Submit button. If successful, you will be able to go back to view your report, but you will no longer be able to edit any fields in the submitted report. If you find any errors, ask Ms. Marte to make the corrections; please DO NOT submit a new report. |
